Background

Every year people drown after disappearing into storm drains during floods. Most of those lost are children and first responders. The most dangerous storm drains have been small pipe openings and culvert inlets with no safety grate covering the opening. Use this form to report areas and collect information where you see open pipes to help MHFD and others determine where these potential hazards exist.

How To Complete This Form

Fill out one survey per location
Fill out each part of the survey to the best of your ability (three parts!)
When prompted, upload your best photos from each assessment location (max of 5 per location)

Safety vest
A measurement device such as tape measure (or distance measurement applications on smart phones)
A phone with internet access
