Historic Reference

This form was originally created in October 2018 to address a gap in the account request workflow. At the time, one-off requests could be made via a form, but bulk requests required a spreadsheet. This form was designed to generate the necessary Excel file for bulk requests.

Over time, the form evolved into an MDOT audit tool, allowing managers to review requests and provide approval before the Maryland Department of Information Technology (DoIT) processed them. The form is now being retired as DoIT has implemented a new request intake process that integrates Esri software licensing into the AGOL request workflow. Statewide, Esri licensing is now managed through individual user accounts rather than the legacy concurrent licensing pool.
